There are two types of medical test:
(1) You can go do a upfront (UF) medical test. For this you need to go to a panel physician before applying for visa with your passport. They will take your test and will give you a receipt contacting IME number. You need to attach a copy of this receipt/IME number with your visa application.
(2) You can submit your visa application. Visa officer (sometimes they do not issue a medical, it depends on your file) will e-mail you a medical examination request followed by a unique medical report form (IMM1017E). You need to take this form with an official ID, to any of the panel physician.

Although this is written for immigration purpose, study permit applicants also go throuh similar tests.
Medical reports will be submitted to VFS the by the Physician panel themselves within two working days. No need to query for the medical report in the test center. This is call e-medical.
Which one is better?
From our experience, we have NOT find any difference for medical upfront or later in the process. In 2016-2017 session, all those who got rejected did UF medical in 50% cases, then other 50% did medical when asked by the visa officer.
However, we suggest that if you are applying 1/2 months before your intended class start time, you should do an UF medical to save time. If you are applying 3/4 months earlier than your start session, you can skip UF medical.
If you have a disease or suspect that you may be inadmissible like probably have TB or HIV, you can do a UF medical to see what the tests are coming out.
What to bring for your test?
proper identification (you will need at least one document your photograph and signature, such as a passport, driver’s licence, or national identity card).
eye glasses or contact lenses, if you wear them.
any medical reports or test results that you have regarding to previous or existing medical conditions.
the Medical Report form (IMM 1017E), if you do not have an up-front medical exam. The visa office will send you this form.
